OFA Eye Clinic Instructions for Online Forms:
1. Go to OFA.org
2. Click on Health Clinics in the drop down menu
3. Log in to OFA online
3a. “Start here” to set up a free OFA account if you don’t have one
4. Click on Health Clinics on left hand sidebar
5. Scroll to the row:
March 7th - 8th Calgary Alberta (orange button) / Calgary Associated Dog Fanciers Kimmi Curley / Calgary Alberta Inglewood Skate Shack
6. Click on the Orange Date to register
6a. To look up a dog already on OFA, use their registration number under "other registrar" -
7. If your dog is already in the OFA system, it will populate the fields – just fill in any missing data
7a. If your dog is not in the OFA system, fill in all the fields
8. Click on the day of the appointment.
9. Click on SAVE/REGISTER
10. OFA will send you an email with the completed form - you do not need to bring the form to the clinic as our team will have access to forms online. After the doctor completes the exam, you will receive an email from OFA with the results and options for submitting.
Results can be found online in your OFA account.

Without the exam starting on your account, the doctor can not do the exam, and the entry will be taken as a donation to the club.
